Quick Answer
You need a deep cleaning if a periodontal probe measures 4 mm or deeper pockets between your teeth and gums. A regular cleaning (prophylaxis) only addresses calculus above the gumline — it cannot reach the root surfaces below. If you have bone loss visible on X-ray plus 4+ mm pockets in 4 or more teeth per quadrant, your insurance will cover SRP. Anything less and you only need a regular cleaning.
A deep dental cleaning — clinically called scaling and root planing (SRP) — is the front-line non-surgical treatment for early-to-moderate gum disease. Florida cost: $200–$400 per quadrant, or $800–$1,600 for the full mouth. This guide covers exactly what it is, who needs it, how to know if your dentist is overprescribing, and what insurance pays.
Quick Answer: Who Actually Needs One?
You need a deep cleaning if a periodontal probe measures 4 mm or deeper pockets between your teeth and gums. A regular cleaning (prophylaxis) only addresses calculus above the gumline — it cannot reach the root surfaces below. If you have bone loss visible on X-ray plus 4+ mm pockets in 4 or more teeth per quadrant, your insurance will cover SRP. Anything less and you only need a regular cleaning.
Deep Cleaning vs Regular Cleaning
Two completely different procedures.
- Regular cleaning (D1110): Removes calculus above the gumline. 30–45 minutes. No anesthetic. $90–$150. Covered 100% by most plans every 6 months.
- Deep cleaning (D4341/D4342): Removes calculus from root surfaces below the gumline. 60–90 minutes per quadrant. Local anesthetic. $200–$400/quadrant. Covered 50–80% with periodontal benefit.
What Happens During Scaling and Root Planing
Local anesthetic numbs one or two quadrants per visit (most patients prefer two visits). The hygienist or dentist uses ultrasonic scalers and hand instruments to remove tartar and bacterial biofilm from below the gumline. Then root surfaces are smoothed (planed) so gum tissue can reattach cleanly. Some offices use adjunctive laser therapy or local antibiotics (Arestin) placed in deep pockets — these add $40–$100 per site and have modest evidence for additional benefit in the deepest pockets.
What Insurance Pays
Insurance covers SRP only if specific clinical criteria are met. The standard codes (D4341 for 4+ teeth per quadrant, D4342 for 1–3 teeth per quadrant) require: documented 4+ mm pocket depths with bleeding on probing, radiographic evidence of bone loss, and a previous prophylaxis within the past 24 months. If your office bills SRP without these criteria, the claim will be downgraded to a regular cleaning and you will owe the difference. Always ask to see your perio chart and X-rays before approving SRP.
Recovery and Aftercare
First 24 hours: Mild soreness, sensitive gums, possible slight bleeding. Soft foods, ibuprofen 400–600 mg as needed. Days 2–7: Sensitivity to cold may persist as gums tighten around root surfaces — desensitizing toothpaste (Sensodyne) helps. Weeks 2–6: Gums shrink and re-attach, pocket depths reduce. A follow-up perio re-evaluation at 6 weeks confirms whether pockets shrank to 3 mm or less. After SRP you transition to periodontal maintenance — cleanings every 3–4 months instead of every 6.
When You Are Being Overdiagnosed
Some offices push SRP on every patient. Red flags: SRP recommended without showing you a perio chart with pocket depths, recommendation includes adjunctive laser therapy or Arestin in every pocket regardless of depth, the office wants to do SRP at your first visit before establishing a baseline, or the cost is significantly higher than $400/quadrant. Get a second opinion if anything feels off — a comprehensive periodontal exam at our Spring Hill office is free with a new patient appointment.
If your gums bleed when brushing or you have not had a cleaning in over a year, schedule a comprehensive periodontal evaluation. We will show you your pocket measurements and X-rays so you understand exactly what you need — and what you do not. Call (352) 597-1100.
